Help!! How to extend schengen visa?

I am a Taiwanese,now staying Italy with my boyfriend who is an Italian. We plan to stay here for 3 months at first,but now we're considering to stay here longer. However accroding the information we checked,it seems I can't extend my schengen visa for more days,and I need to go out of Europe for 6 months in order to apply for a new visa in my country.
So the question is:Is there really no other way to extend my visa without leaving Italy?
For example:
My boyfriend's uncle has a business,is that helps if he hires me as an employee?
or
I need medical care in Italy?

Please provide any kind of information~
