HEALTHCARE IN MEXICO
OVERVIEW
Mexico's social security program provides health care to workers and their families, for some 50 % of the national population (1995). Due to demographic growth (97.5 million in 1999) the State is aiming to provide basic health services for all.
The number of Mexicans with no health coverage has decreased from 10 million in 1995 to 1.5 million in 1999, according to the Pan-American Health Organization. The World Bank, one of the project's sponsors, has contributed US $310 million to the effort. Two of the main obstacles to improving public health services for Mexico's poor have been the population dispersion in rural areas and difficult geographic conditions in reaching them.
Health expenditure in Mexico in 1996 amounted to 4.12% of GDP, US $141 million. Public healthcare costs accounted for 2.12% of GDP, US $ 73 million and private healthcare expenditure amounted to 2% of GDP, US $68 million.
The Mexican Institute of Social Security (Instituto Mexicano de Seguro Social--IMSS) covers approximately 80% of these beneficiaries (all employed in the private sector). The Institute of Security and Social Services for State Workers (Instituto de Seguridad y Servicios Sociales para los Trabajadores del Estado--ISSSTE) covers government /state workers and accounts for 17% of the beneficiaries.
The Secretariat of National Defense (SecretarÃa de Defensa Nacional), the Secretariat of the Navy (SecretarÃa de Marina), Secretariat of Air Force ((SecretarÃa de la Fuerza Aérea) and Mexican Petroleum (Petróleos Mexicanos--Pemex) have their own health programs, which cover military, air force and naval personnel, and petroleum workers, respectively.
In 1956 the Mexican Federal Government bought most of the shares of Mutualista de Mexico, General Insurance Company Ltd. and Aseguradora Hidalgo and amalgamated to become the National Insurance Institution. Aseguradora Hidalgo looks after the administration and services of life assurance for public sector workers and later, provided them with a comprehensive system of social insurance.
Aseguradora Hidalgo implements medical insurance to complement the existing medical cover from the State Social Insurance for workers from Inst. Mexicano del Seguro Social (IMSS) or Inst. de Seguridad Social al Serv. De los Trabajadores del Estado (ISSSTE). This new medical insurance aims to provide specialised immediate and low-cost medical assistance.
There are two methods of Health insurance:
* Compulsory insurance, for State/public, national defence and corporate workers.
* Voluntary insurance, for the self-employed (professionals, farmers, etc.) Their only option is to buy private insurance.
Children are covered up to the age of 16 or up to the age of 25 if in full-time education.
The market for private health insurance is new and the population is just beginning to choose various types of medical cover on offer. Mexicans may choose to complement their social/State insurance plan with private health insurance. The type of cover varies according to the various plans on offer by different insurance companies. Residents can choose to completely replace the state insurance coverage with private health insurance if they so wish, however, it is possible to have double medical insurance, i.e. private and state funded. Private health insurance does not offer cover for alternative medicines; the only exception is medically trained chiropractors registered with the Mexican Medical council.
In 1997 there were:
* 2,172 private medical centres
* 7,418 private clinics (3,049 general clinics and 4,369 specialised clinics)
* 1,305 emergency units (private)
* 244 ambulances
In-patient and Out-patient services cover varies depending on the insurance company and type of plan chosen. There are well-equipped private and semi-private clinics and hospitals, and qualified professionals who provide first class services to the private insurance sector. Dental benefits are optional, but dental emergencies are covered.
Health Benefits for Expatriates
Expatriates are not covered by the Social health insurance while in Mexico. However, they can avail of private health services if medical cover is arranged by their own company or individually. Service providers to the private health insurance sector are usually top of the range medical centres and clinics and there exists a listing of qualified practitioners, centres and clinics per province/district.
Social Healthcare Services
The social healthcare system is comprised of primary healthcare units and secondary healthcare services. In 1999 the entire social healthcare system incorporated:
* Medical Units: 17,634 (16,703 out-patient units and 931 in-patient units)
* Doctors : 128,128
* Nurses : 176,459
