You could go onto www.packet8.com and get their little modem, including one month's usage for $29 USD.
After, you only pay $21 USD per month, tax included, for unlimited calls to North America. For $35 USD (os something like that) per month, you can get a EuroAsia plan where you can call practically anywhere free.
You get a USA number, which if anybody calls you, it rings wherever you are. You don't even need a computer. Only a DSL connection. You can easily hook up their unit, and any plain land-phone in any internet cafe around the world, and it will still work.
Call waiting, call forwarding, message service, many other features...all included.
No prepaid required. They merely bill your credit card of choice $21 USD per month, with no additional charges.
